Our Fort Worth Fossil Creek center is located in North Fort Worth in the Park Glen Market Place at the intersection of Basswood Blvd and Park Vista Blvd.

We know that injuries are never scheduled, but relief can be. If you're injured, tired of having less mobility or experiencing aches and pains, let our licensed clinical experts help.  

We provide comprehensive outpatient physical therapy to individuals suffering from injury or pain associated with sports injuries, back/neck pain, joint problems, balance disorders and work-related injuries. Our therapists work with you and create an individualized treatment plan to help you quickly return to daily activities, work duties and recreational pursuits.

Our Team

Rachael Cook, P.T.,DPT - Center Manager

Rachael Cook, P.T., DPT brings a high level of curiosity and compassion to the patients she treats. She was born and raised in Bedford, TX and returned to the area after a short time on the road as a travel therapist. Rachael enjoys treating patients of all ages and ability levels, is experienced in orthopedic therapy and has a passion for promoting an inclusive definition of wellness. Outside of the center, Rachel enjoys spending time traveling, running, baking and cooking, training service dogs and streaming whatever is new on Netflix. You'll probably find her with coffee in one hand and a book in the other on days off. Rachel's professional areas of interest include running-related injuries, education, nutrition and pain science. She is also trained in Trigger Point Dry Needling with Baylor Scott & White Rehab.
